چکیده
PDN is commonly caused by involvement of either the C fibres or the Aδ fibres. PDN caused by the involvement of small nerve fibers (C fibres), may present without objective clinical findings, such as decreased peripheral reflexes or abnormalities, on routine electrophysiological studies. Small fiber neuropathies may manifest as a number of different clinical symptoms, including allodynia, burning pain, defective warm thermal sensation, and defective autonomic function, e.g. decreased sweating, dry skin, and impaired vasomotor control. Large fibre (Aδ) pain is deepseated gnawing, dull or cramping with impaired vibration perception and position sense, depressed tendon reflexes, and sensory ataxia with marked electrophysiological abnormalities.

Amitriptyline 2% cream vs. capsaicin 0.75% cream in the treatment of painful diabetic neuropathy (Double blind, randomized clinical trial of efficacy and safety)
Because of less systemic side effects of topical medications in pain relief of the painful form of diabetic peripheral neuropathy, this study aimed to compare the effect of amitriptyline and capsaicin cream in relieving pain in this condition. In this randomized, double-blind and non -inferiority trial, 102 patients received amitriptyline 2% and capsaicin 0.75% creams 3 times a day for 12 week...

متن کاملLocal sympathetic denervation in painful diabetic neuropathy.
This study assessed whether painful diabetic neuropathy is associated with abnormal sympathetic nervous function in the affected limbs. Nine patients with diabetes (four men, five women; age 61 +/- 7 years) and painful peripheral neuropathy of the feet, but without evidence of generalized autonomic neuropathy, underwent intravenous infusion of tritiated norepinephrine (NE) and sampling of arter...
متن کاملPrevalence and Characteristics of Painful Diabetic Neuropathy in a Large Community-Based Diabetic Population in the U.K.
OBJECTIVE To assess, in the general diabetic population, 1) the prevalence of painful neuropathic symptoms; 2) the relationship between symptoms and clinical severity of neuropathy; and 3) the role of diabetes type, sex, and ethnicity in painful neuropathy. RESEARCH DESIGN AND METHODS Observational study of a large cohort of diabetic patients receiving community-based health care in northwest...
متن کاملSympathetic Blocks Provided Sustained Pain Relief in a Patient with Refractory Painful Diabetic Neuropathy
The sympathetic nervous system has been implicated in pain associated with painful diabetic neuropathy. However, therapeutic intervention targeted at the sympathetic nervous system has not been established. We thus tested the hypothesis that sympathetic nerve blocks significantly reduce pain in a patient with painful diabetic neuropathy who has failed multiple pharmacological treatments. The di...
متن کاملPregabalin versus Gabapentin in the Management of Painful Diabetic Neuropathy
Diabetic neuropathy develops in the majority of poorly controlled diabetic patients as a late complication. Painful diabetic neuropathy represents a major health problem as it may present with excruciating neuropathic pain and is responsible for substantial morbidity, increased mortality and impaired quality of life. The aim is to compare the efficacy of Pregabalin and Gabapentin in painful dia...
